How does sinq identify Care Gaps?
Sinq can work with health plans to integrate existing Care Gaps if the organization maintains information. Alternatively, sinq can partner with health plans to integrate basic claims data to derive Care Gaps using sinq’s processing engine and agreed parameters. An example of this would be using claims to identify when the last primary care visit occurred and whether a Care Gap to schedule a new PCP visit exists.
The result is providing our clients with the ability to both identify and select which types of Care Gaps to circulate at that particular point in time based on any business considerations (STARS / HEDIS, targeted campaigns, etc.).
